Treatment with hydroxocobalamin and betaine is effective in preventing progression of kidney disease and recurrent TMA. Immune-mediated drug-induced TMA is due to an antibody (Ab) formation against a drug or its metabolite. Eculizumab And Other Therapies For The Treatment Of High-Risk Transplant-Associated Thrombotic Microangiopathy
Isolated TMA allows to continue the drug under tight surveillance, in the absence of an alternative cancer therapy. However, the presence of ATN is a formal indication to stop the drug immediately 64. We established criteria to assess the strength of evidence supporting a causal association of a drug with TMA and systematically identified and reviewed all published reports of DITMA. Our goal was to assist clinicians in their evaluation of patients with suspected TMA by developing standardized criteria for assessing clinical evidence. This form of TMA can be seen with mucin-producing adenocarcinomas as well as disseminated malignancies. What occurs is microvascular obstruction with cancer cells, consumption of platelets in tumor microthrombi, and fragmentation of the passing red blood cells.
Narsoplimab Treatment And Response
Patients who initiate early chemotherapy have a better prognosis, with a considerably higher survival rate and a good quality of life. Plasmapheresis could be useful if antibody-mediated TMA is suspected 10,142. A cross-sectional multicentre study 143 showed that patients with TiTMA, when compared with those with idiopathic TMA, had a longer duration of symptoms, including some atypical ones, such as bone pain and wasting. To sum up, even if no etiological role has been recognized with certainty in patients with HSCT-TMA, it is clear the association between endothelial damage and complement overactivation, both in early and terminal pathways. Endothelial injury is also a feature of GVHD, with endothelial cells as the putative direct target of donor cytotoxic T lymphocytes 88,106,111.
C4d staining is used in the assessment of antibody-mediated rejection in kidney transplant biopsies and is a marker of the classical complement pathway activation. In a study with 20 children (8 of which had TMA), arteriolar C4d staining was more commonly found in patients with HSCT-TMA (75% versus 8%), which could represent complement activation by recipient- and donor-specific antibodies 8,107. Additionally, significantly higher CH50 levels were described in HSCT-TMA patients 104,120. The association with mitomycin was only possible in 54 patients, principally because it was often administered together with 5-fluorouracil.
Breast-conserving surgery was performed 7 weeks after termination of eculizumab, followed by radiation therapy. During 47 months of follow-up, renal function continued to improve (eGFR 68 mL/min), and no relapse of TMA has occurred (Figure 1A) (Table 2). In contrast, HUS most often occurs when your body is exposed to a certain type of toxin (Figure 4).

Team 1: Primary TMA Versus Team 2: Secondary TMA

Alternative Pathway Of Complement Activation

Worse outcomes were described in patients with higher sC5b-9, with a lower likelihood to respond to treatment 103. More recently, Jodele et al. 103 showed a 50% rate of complete response to eculizumab, with severe disease being less responsive, which could suggest the existence of other targets of endothelial injury pathways. Besides, eculizumab comes with several challenges, namely the cost, timing of initiation, patient selection, dosing and duration of therapy 93,102. The moderate therapeutic success achieved with eculizumab has led to investigation of other targeted therapies that act on various stages of the complement system. Some studies, still on phase II/III, already show encouraging results 93. Several studies evaluated plasmapheresis efficacy in this setting, with controversial evidence.

TMA should be suspected in patients who develop new-onset or exacerbation of HTN, requiring more than 2 anti-hypertensive drugs. Systolic HTN has been appointed as an early marker for TMA, particularly in children 8,95,105,122. Immediately after HSCT there is a high risk of AKI, with risk factors attributable to the procedure and the drugs administrated. Serum creatinine levels will probably not provide a reliable estimation of GFR, because these patients may display large fluctuations in nutritional status, weight and muscle mass 91,97.
All patients except two pediatric patients with sickle cell disease underwent HSCT for hematologic malignancies. When antibody-mediated TMA is suspected, a trial with plasmapheresis could be useful. However, some cases only respond with complete malignancy remission 141.
ADAMTS13 Tests
The drugs reported as causative of TMA are heterogeneous in terms of mechanism of action. Some drugs are more frequently used in clinical activity, as antibiotics or antiplatelet therapy, some others in more restricted contexts, such as IO-anti-VEGF. The heterogeneity of these causative drugs should require that physicians from several different areas are familiar with DITMA. Thrombotic microangiopathy with a primary finding of kidney failure is termed hemolytic uremic syndrome (HUS). The most common form is HUS following infection with Shiga toxin–producing Escherichia coli (STEC), which is typically accompanied by bloody diarrhea (table 1). This form is called EHEC-HUS (enterohemorrhagic E. coli, EHEC) or STEC-HUS.
One-hundred-day survival after narsoplimab treatment was of 70% and the median OS was not reached. All the responders achieved 100-day survival (100%) and, after a median follow-up of 13 months, are alive and in remission. Narsoplimab has been recently investigated in a phase 2 study for the treatment of different patterns of microangiopathy and was converted into a pivotal trial for TA-TMA only upon request to the U.S. The pivotal study showed that narsoplimab was safe and effective in the resolution of clinical outcomes and laboratory markers of TA-TMA with a benefit on survival 24.

A 2016 review of research suggests that TMA originating from cancer treatment drugs may be more serious and that the prognosis is more encouraging when TMA develops as a result of the underlying cancer. In most patients with TMA, a complement-activating trigger can be identified, and in 28% of patients with an activating trigger, a genetic risk mutation can be found 3. In agreement with others we found that TPE/plasma infusion was not effective in patients with chemotherapy-induced TMA 12, 13, 14, 15, 16, 17, 18. Thrombotic microangiopathy (TMA) encompasses a group of disorders presenting with microangiopathic hemolytic anemia (MAHA), thrombocytopenia, and ischemic organ damage, most frequently of the kidneys and the central nervous system 1, 2. In many of the more atypical TTP/HUS disease patterns the optimal treatment has not yet been standardized.
